Using Insurance_2003.mdb database, create the following reports

1. qryEmployeeListByLocation – Alphabetical phone list of employees by location, with hire date

2. Fix the column headings as shown below. Add a report Title header and date/time footer.

Create a list of prospective bonus recipients using “Excellent” performance as your criteria. If the performance is excellent, propose an award of 10% of base salary. Include a location subtotal and grand total of the proposed award program.  Produce a report similar to the one below.
